Also, consider downloading Valley since it's around 400MB. Look for other tasks or processes using your GPU besides the game—Task Manager can help you identify them.
In the test it displays my card as RX570, though here it shows RX 480.
You can disregard that idea. Your score is below 570, which isn't too concerning. From what I see, everything seems okay. I believe your GPU is in good shape.
