I managed to fix the issue by using one of my other monitors as an IGPU, connecting it properly. Now two monitors are linked to the IGPU from the CPU, and the third one is connected to the GPU. This setup seems to be a limitation of the GPU rather than something else.

The overall bandwidth and resolution cap on display outputs is a consistent factor. Nvidia GPUs and the RTX 4000 series share this restriction too. Otherwise, there might be attempts to push 16K at 240Hz or higher across several monitors.

The refresh rate impacts overall bandwidth too, since switching one display to an IGPU left me with two connected units, which made the 360 Hz setting work well.

The faster the refresh rate, the more information is being sent to the GPU. For example, a table from the Wiki and the RTX 4090 specifications show this detail.

I successfully got it running as expected. Set up your high-end monitors on the GPU with 144Hz, 24Hz, then added the 165Hz, 27Hz, and finally the 360Hz, 27Hz. On the IGPU, configured two lower-end monitors at 60Hz, 24Hz, 1080p.
