My guess is the hdd as well. Sounds like it's having trouble keeping up. Also, sometimes Diablo just lags when there's stuff on screen, it even does it to me with a 4670k @4ghz and 760gtx. Warcraft was famous for horde lag as well, when someone else popped into your area the game would lag.
