Attempted several fixes before dismantling it to check compatibility in various PCI slots. Results remain unchanged, indicating potential GPU issues. Planning to visit a local PC repair shop or store tomorrow to have it tested on their systems before returning it for an RMA.

I brought my PC to a repair shop today and had their GPU tested. Everything functioned properly, but they couldn't get my card to work. It seems I might have been unlucky and received a faulty GPU out of the box. I returned it to the store where I bought it and received a full refund.

It's disappointing, but good luck with the return process. Please let us know the status. Occasionally these can be frustrating. I resolved the issue by placing it in the lower PCI-E lane—now it functions properly each time.
