It crashes frequently and I only achieve 30 to 50 frames per second at 1080p.

It might be anything—your CPU, GPU, RAM, or some random software you've added. See if lowering their speeds resolves the issue. If it works, consider reinstalling Windows without all the current installations and test again. For improving performance, look into Hardware Unboxed's optimization guides on YouTube.

Also, note that your hardware doesn’t meet the required minimum specifications for this title, so achieving 30 to 50 FPS at medium settings on 1080p is actually quite solid from a practical standpoint.

I've been playing for more than a month as a R* launcher buyer, and the version that causes the most crashes is 1205 or 06. The crashes happened every 5 or 10 minutes, which was really frustrating. NVIDIA had to release driver 441.34, and R* received two patches to fix the issues. Once I switched to build 1207, I can play for hours without any problems—no crashes, no stutters, and consistent 60 fps.
If you're already on the latest version, run Afterburner and provide detailed stats like CPU, GPU, RAM usage, temperatures, and clock speeds at the crash points.
