I've been using it for roughly a year, typically once or twice a month. There have been no problems reported, so it seems to be safe.
You can export the registry, apply it, and then resolve any issues by restoring it. If problems arise, simply reinstall and use registry cleaners to remove outdated or unnecessary files such as invalid startup entries, obsolete firewall rules, and similar items.
I rely on it each week (I’m very cautious about cleaning my computer) and I’ve never faced an issue, never backed up anything. If you’re worried, I’d grab a small USB drive—at least 1GB—and when CCleaner suggests backing up, just copy the files there and label them accordingly. Edit: I recently checked the file size and they were only a few KB; for total savings, even a 512MB flash drive would work fine.

CCleaner's registry tool is reliable. It mainly eliminates unused extensions and outdated dependencies in the registry. It’s fine to handle manually and remove keys you no longer require; registry cleaners won’t take everything off. In my opinion, I’d have simply reinstalled Windows and used Lubuntu instead.
