I ran both sfc/scannow and DDU with driver reinstalls, but the issue remains. I disconnected all USB devices except my mouse and keyboard to check if the "USB video adapter" problem persisted, still causing blue screens. I also attempted advanced display settings with only one monitor connected at a time (using both monitors), but the blue screen continued. I’m hoping the latest BSOD dump will help identify the cause.

It remains ddkmd.sys. I searched further and found it relates to displays not linked directly to the GPU. The manufacturer also offers tools for using your phone or tablet as a monitor. I’m unsure if the same driver applies, but you might want to uninstall any available tools. There’s no hub or dongle with video outputs connected. Just press Windows key + R and type "appwiz.cpl" to check for "Duet", "iDisplay", and "Osbase".

This was the answer and I hadn't considered it when you mentioned adapters, as I assumed they were physical devices. About eight years ago, I used an app called "Duet Display" to turn my iPhone into a second screen, but I never revisited it. After uninstalling and restarting, I accessed advanced display settings and adjusted the refresh rate to 170. I recognized it immediately when you mentioned manufacturers offering phone monitoring tools! Thank you—this would have surprised me completely.
