Could your HD audio cable be deteriorating after five years of use? That might explain the issue.

Some part of my concern about disconnecting it, though I'm unsure if it affects the rear IO.

I didn't notice that because I typically connect my blue Yeti headphones via USB, making the HD audio cable irrelevant.

The problem occurs with the 3.5 mm jacks both in front and back, the USB adapter, but not with the HDMI output. Also, does this affect the headphone jack when using HDMI to a monitor?

Next, don't assume it will function... have you cleared the CMOS yet? Have you removed the CPU from its socket or taken out the RAM during the time this problem occurred?

Reset CMOS but nothing changed. I haven't altered anything inside the computer until the issue began.
