I purchased the card from an online store, similar to a prior mention where many x540 and x520 counterfeits appeared on eBay. I’m hoping it wasn’t bought directly from China.

If your card lacks a YottaMark label, it might be counterfeit or produced by an OEM supplier such as Dell, HP, or SuperMicro.
