I encountered similar issues as well. Be sure to click everything. Navigate through the menu, select the bestiary, choose the entry, and then exit. It might need to be repeated a few times.

It appears right in front of you when you enter the main gate of the Abandoned Village, near the north entrance as indicated on the map above. Head inside and use your Witcher instincts to spot clues around the well and within the ruins. A journal is likely hidden there—check it out. You might receive updates about this quest once you find it. I don’t think reading the Bestiary is mandatory, but it’s worth noting that the Devil by the Well shows up separately in the Bestiary (alongside the Noonwraith entry), which suggests you haven’t triggered it yet.
