Windows.old holds a backup for when you upgraded to a newer Windows release (such as 20H2 for Windows 10). Removing it means you lose the ability to revert to the previous version. It stays intact for about 30 days before it gets removed automatically.
On Windows 11 you can remove it via Settings > System > Storage, tap Temporary files, look for old Windows files, and click Remove files at the top of the list. The reason behind this is explained below.
