Even at 4.2ghz with 1.2vcore, performance isn't stable. Could it be the chip itself or something missing in the setup?

1.2v is reduced, but the safe remains secure up to 1.3v provided heat isn't an issue

Wat betreft PSU, het kan zijn dat het niet genoeg watt levert. Of je kunt proberen de spanning iets hoger te verhogen.

It operates smoothly in the high 70s at 1.2, though I could attempt it. The EVGA Supernova 650 G3 is psu stable at 4.1 with 1.15, but nothing exceeds that. I experimented with going up to 1.25 for 4.2, but still encountered boot issues.

what kind of cpu cooler are you using?
as mentioned before, the cpu can manage higher voltage with a good cooler.
*edit* corrected earlier mention about the cooler in the first post.

Typically, maintaining stable temperatures is important, so keeping the CPU below 1.35v is advisable. It's possible your 4690K doesn't handle overclocking well, but experimentation will clarify this. Additionally, refreshing the cooler with new thermal paste could be beneficial if temperatures rise unexpectedly.
