Are you on the latest BIOS revision for your motherboard? Your wall of text doesn't include the make and model of your PSU and the version of your OS. Did you recreate the bootable installer for Windows 10 using Windows Media Creation Tools?

No. I haven't flashed to the latest revision.
PSU is EVGA Supernova 550 G2.
No. I used the Reset tool to format and reinstall.
