The Molex connector isn't attached; these devices usually need the Molex plug to supply USB power.

I don’t recall plugging in a Molex cable to this USB PCIe extender. It might have been damaged before, but I didn’t notice anything unusual.

By the way, why is the USB card in the X16 slot? Was the PSU replaced by yourself or did a technician handle it? You might need to reinstall the driver.
