I know sometimes the measurements aren't exact, like CPU temperatures displaying 127°C and 103°C. Should I worry about a 20.736V reading on the 12V rail? What actions can I take to confirm or fix the issue?

You chose the incorrect test location. A +20 would damage the MB and other components. HWinfo can handle this situation.
