Once I powered up the PC, I noticed this strange display. It appears to be the monitor issue, as changing it off and on fixed the problem (no cables touched or PC restarted). Could this be a faulty panel, GPU, or driver problem?

My specs:
Motherboard: MSI MPG Z490 Gaming Plus (MS-7C75).
CPU Cooler: Noctua NH-D15.
Case: 275R Carbide, Tempered Glass, Black.
Processor: intel core i7 10700k.
GPU: MSI 4070 Ti Super Expert.
Power Supply: DeepCool DQ850 Gold (2.5 year old).
RAM: 32 GB 3700 MHz DDR4.
Drives: Samsung SSD 850 Pro 256 GB; WDC WD1003FZEX-00K3CA0 (1 TB HDD), Team MP33 (1 TB SSD).
OS: Windows 11 25H2

Does the distortion occur each time the PC is initially powered up? What is the typical sequence for turning on the PC, monitor, and any other connected devices? Is it possible to verify the monitor's functionality on a different known working setup? Or attempt another verified working system on your own? Identify whether the issue relates to the monitor or persists within the system. More details are required.

Does the distortion occur each time the PC is initially powered on? Currently, I can’t recreate it by restarting the system. The PC started up without any problems today.

What is the typical sequence for turning on the PC, monitor, and any peripherals? I generally prefer a cold boot method. We have connected a USB hub to the monitor, but it has been used for some time without any issues noticed. The normal order of activation is: 1) PC fans; 2) keyboard; 3) monitor and USB hub; then BIOS logo appears, followed by the HDR symbol in the upper right corner of the screen, and finally the Windows lock screen. After that, there’s a brief pause of 1-2 seconds before the NVIDIA control panel adjusts its color settings automatically.

Can you confirm if you can test the monitor on another known working system without distortion? Sorry, I don’t have access to another system at the moment.

Would it help if you tried using another known working PC (without distortions)?

I also have another monitor, which will be installed in a few days. I tested the GPU and other hardware components, and played Diablo 4 at maximum settings for six hours. The average temperatures remained around 50 °C for both GPU and CPU, with a hotspot difference of 9.4-10 °C. Everything ran smoothly, without any stutters, maintaining a solid 155 fps.

This is my third monitor in the past 12 years, and this is the first time I notice such image distortion. The device has a 6-month warranty, but I still need to figure out how to reproduce it to be eligible for an RMA. I believe taking a screenshot next time could help determine if the image appears broken on that device.
