All games lose sync without VSync, I also try adaptive in GTA since it causes a lot of stuttering.

It means that with Vsync turned on (not adaptive), when your frame rate drops under 60 FPS, the display still shows a lower number—like 48 FPS—instead of immediately dropping to 30 as it would with standard Vsync.

That's correct, VSync limits the frame rate to 60 FPS. If it drops below that, it won't adjust between 30 and 60; it just stays at whatever value it is set to.

Since it uses triple buffering, a downside is minor stuttering when the display refresh rate drops below your screen's speed.

Adaptive Vsync activates VSync only when your FPS goes over 60FPS, while keeping it on even if your rate drops under 60FPS, without impacting performance much.
