Would it help if you provided a screenshot? Also, note that the rear fan seems to be malfunctioning—it might be dead. Make sure to check if the fans on the graphics card are spinning properly.

4% to 12% load per core and temperatures between 75C and 78C? Your CPU is overheating.
Your CPU cooler seems inadequate—consider upgrading to something like Deepcool AK400 or Cooler Master Hyper 212.
Have you checked the fans on your graphics card? Are they spinning? Is this your graphics card?

the fans seem to be handling the GPU issue, and it appears this is my GPU. just got the Deepcool AK400, hope it solves the problem.

This occurs often when opening games; initially everything works fine but then the frame rate drops, possibly due to increased GPU temperature.

Your rear exhaust fan has died.
Your pc doesn't have any intake fans.
There's simply no airflow.
Your pc case is not meant for gaming grade components.
Can you show photo of your pc from front?
You'd probably have to get a new modern pc case also - with mesh front and proper airflow.
