PC black screen crash with numerous nvlddmkm errors
Recently my pc started displaying a black screen unexpectedly during gameplay. I attempted to monitor temperatures and while the GPU was running at high levels in the 90s, it didn’t match any specific crash moments (no spikes). In the Event Viewer there are hundreds of nvlddmkm errors, some with code 13 and others with code 0 (attached image shows this, errors keep appearing as I scroll). In reliability history, a LiveKernalEvent 141 appears at each crash time. When it crashes, both monitors go black, indicating signal loss, fans speed up, and I can hear audio briefly before the system forces a shutdown.
Windows 11 (recently updated but issues persisted with Windows 10)
Have you upgraded to Windows 11 via the internal upgrade method? If yes, reinstall the OS after generating a bootable USB drive. Install the OS in offline mode. Then, manually install all necessary drivers using the latest version through an elevated command—right-click the installer and select Run as Administrator.

What is the make and model of your case? How many fans are there in the case, and what is their orientation?
I made the switch to Windows 11 via the update window in the control panel, even though the issue existed before the update. I’m using BIOS version 1654 with a Lian Li 011 Dynamic (O11DW) case, running 12 fans total—9 intake (with push-pull for my AIO) and 3 exhaust at the top. I intend to replace the case, but these fan numbers seem too many and noisy.
