Typically this happens because the screw is more firmly lodged than the standoff itself. I've encountered this when screws were cross-threaded, yet the manufacturer still used them. Secure the standoffs at their hex connection using tools like pliers, a wrench, or a vise, then attempt to pull the screw out again.
