We have affordable onpy tools to verify voltage when no load is applied and if it activates. The most probable issues are a damaged board or a faulty power supply unit. I’ll replace the PSU to check if it restarts—then power supply failure is likely the cause. Power spikes can cause immediate harm or short-term damage, which isn’t uncommon; often the PSU is the culprit.

Lack of backup PSUs means only one unit was tested. The chance the motherboard suffered damage while the PSU remained functional depends on the components' quality and conditions during use.

Typically improbable. Fas as I recall your PSU is decent, so it should have handled some overcurrent protection. Not usually strong enough to completely stop a surge, but often enough to harm or destroy the PSU along with other components.
