The issues stem from driver problems since last year, as mentioned in the quoted post. I’m significantly impacted and might need to switch to the Redux version.
It seems it depends on your setup and comfort level. If you're used to playing without VSync and aren't bothered by small stutters or tearing, you might enjoy it. For those who prefer a locked frame rate, microstutter can be really bad. I’d actually favor playing 2033 on the Xbox 360 over the PC.
I just played it recently on a 560ti rig and my 760, and microstutter was not an issue While the engine isn't perfect it was easily the best graphics of a game launched in 2010, and the game in general was amazing for the budget it received.
I play every game using V sync since I dislike breaking immersion. Metro 2033 performs well on my current setup, which is quite standard for mainstream systems. It also improves its performance significantly when using two GPUs together, especially in high-end configurations.
I'm not sure what to say then—maybe it's a problem with specific cards. For me, it was tough playing at all. Even on a 780ti, the game felt much worse, with serious stutters and far less content than the Metro 2033 redux version. I wouldn't suggest anyone buy the older game anymore since the redux is no longer available.
The original version offers superior visuals. However, I’d definitely disable Physx since it’s quite unstable and doesn’t use four times MSAA—just enough for smooth playback unless you prefer a slideshow feel.

Playing Crysis 1 feels superior compared to many current games today.
I've experienced only minor problems when launching the original Metro 2033 and LL on my older hardware. I ran it smoothly on a triple HD7970 (back then) and a dual R9 290X, as well as on a dual GTX670 in SLi. There were occasional small hiccups, but they didn’t disrupt gameplay. My triple HD7970 handled the issue well—almost unaffected by the micro-stuttering some reported with dual cards. I was even able to play Metro LL at full 1080P on my second rig without any problems, and the difference between Advanced PhysX enabled and disabled was barely noticeable, staying within five frames.
