New programs can't be added if they weren't recently shown. Unpinning them always restarts the list from the start (before any customizations).

Use a restore point or a USB with the Windows 10 installer to fix your installation. Check if the issues persist. For SSDs, optimize the disk using the utility. On HDDs, run a defrag.
