I would install MSI Afterburner, adjust the fan speed manual, and attempt to make the fan vibrate. Use your finger near the blades without inserting it, and stop the fan once it starts shaking. Identify which fan is affected. Avoid doing this frequently to prevent damage; replace the faulty fan if necessary.

I know it is the right fan, how easy is it to replace and also are there any chances that It could be a sagging gpu?

GPU is sagging, do you have some Lego at home? Try crafting a stand. I’m not sure how probable it is. I can only suggest building a stand and testing it. With MSI afterburner you can lock the fan speed. It makes a rattling noise when you use the stand, then it stops. Stand fixed. If it keeps happening, consider replacing the fan. Swapping isn’t too tough; it’s not clear if it’s the same model as your GPU but there are plenty of videos. By the way, what I did with my current old GPU is...
