In GTA V the game frequently freezes and displays err_gfx_d3d_init errors, mostly during missions or when events occur, such as shooting a guard who happens to have an access key. It also appears randomly. This issue seems to appear only in GTA Online. I've tried several fixes: reinstalling the game, adjusting overclocks, disabling overlays, turning off Vsync and Tessellation, switching to DirectX 10.1, reinstalling DirectX files, updating GPU drivers, and increasing the pagefile size from 4GB to 8GB.

navigate to C > Windows > System32 and find the d3d9.dll file, then place it in your main GTA5 directory.
