So yeah. I played the game four days ago and it's the second time I encounter a black screen and have to restart. Usually when I adjusted the settings in the Video menu and after the cutscene following the death of Faith (after talking to the Sheriff), everything turned black, the coolers stopped working, there was no sound, and I had to restart manually.
In the Windows event log, I got a critical kernel error code 41. This happened for the second time.
I tried setting the CPU to a fixed voltage of 1.27V, SA voltage at 1.34V, and RAM voltage at 1.34V because the automatic voltage settings are too high [1.4V]. I’ll switch back to auto and test with the results to see what happens.
My question is – is this a game issue or a hardware problem?
It's probable the auto OC, including manual ones, needs careful handling and may not work in every game. I'd consider using default clocks but ensure your RAM is set to XMP mode for optimal performance. FC5 should operate smoothly at full settings on that system without any modifications.

The core voltage is the key factor that auto OCs often struggle with. They tend to apply it much higher than necessary. That adjustment probably resolved the issue, and you're appreciated.
