This appears to be a personal issue since no one else is experiencing the same problem, so it isn't a game bug. I've spent about 120 hours on Fallout 4 and the only issue I've encountered is my weapon vanishing once, which was resolved by reloading my save. It seems likely driver problems or needing Steam validation if you're using that version.
