It seems there could be a driver problem. This card is quite outdated, and official Intel WiFi/BT drivers support it only up to this point. Could you tell me which operating system you're using? And what driver version are you running?

I just discovered a more recent version for my card, which I updated to 15.11.0.9, but I'm still having trouble connecting via Bluetooth.

Yes, the Bluetooth adapter appears in the Device Manager.

These steps aren't very useful—they come with the included drivers. If it isn't visible in the control panel, it's likely not functioning properly. It might be because the mPCIe to PCIe adapter doesn't recognize the transfer or is faulty, but I'd lean toward the former. Have you used this card in a laptop or another card with BT in the adapter?
