Increase the voltage or lower the clock speed. My 3570k stable worked fine on Prime but not on BF3; adjusting by +0.005V fixed it.

I suspect the overclocked GPU might be the main issue, especially since Far Cry 4 demands a lot from the graphics card.
I’ve already boosted my GTX 770’s speed before, but it became unstable in most of my games.
I can push my 2500K to 4.4Ghz at 1.245V. I know voltage changes affect performance, but raising it further probably won’t solve the problem.
If you reset the GPU to its normal settings and keep the CPU overclock unchanged, what would that result in?

It's unclear exactly what's happening, but the issue seems to appear roughly after 40 minutes. A power supply could potentially be involved.

Try playing Far Cry 4 without enabling the GPU overclock and observe the outcome. If it still fails, reinstate the GPU overclock and turn off the CPU overclock to determine the next step.
Troubleshooting involves systematically narrowing down the issue.
