Air remains trapped inside an AIO, making avoidance nearly impossible. Usually the air stays near the radiator and shouldn't create problems. This is why placing your pump at the lowest point in the setup is crucial. It stops the air bubble from traveling down into the pump. In this case, someone moved the equipment physically, likely bumping it or positioning it so the bubble entered the pump and caused dry running. Adjusting the system to relocate the bubble back to the radiator fixed the problem.

As noted, there is always a presence. Over time, certain liquids will seep out via the tubes, increasing the volume of air within the system.
