Edzel creates it with After Effects. The animation is quite basic, but the idea itself is innovative.

I'm not sure if Premiere offers the same flexibility, but it's possible. Another option is to design the image first, then bring it into your project for seamless integration. I think I could mimic a similar effect in about five minutes or less.
