At first I limited my FPS to 60 and it stayed there. Skyim works the same way—just switching the refresh rate from 144hz to 60hz caps the FPS. I modified some settings in the .ini files before stopping playback. Now I’m getting headaches when the cap drops to 30fps. The game speed is tied to FPS, so restoring the higher value should fix it.

FPS above 60 can slightly increase the game's physics speed. For capping, tools like Rivatuner or frame caps are available. You can also find similar solutions at NexusMods for Fallout 4.

Remove the ini files from the documents folder and start the game. This will rebuild them, but any modifications won’t be saved.

Set the resolution to the highest your monitor supports and change Vsync to half. This helps because the game runs at maximum speed, which can cause sync problems. On a 144Hz monitor at 144 FPS, this should stabilize performance around 72 FPS without issues. Let me know if it works for you!
