Check the bottom of the page for the ports you've sent along. It seems the issue might relate to forwarding the same port to another IP address.

Your device's IP address could be fluctuating. Verify that the machine hosting the MC server maintains a fixed IP and that this matches the one listed in the port forwarding settings.
