I've performed a reset of the drivers, enabled a clean boot, adjusted underclocking, limited power usage, and cleaned out the GPU. I'm not sure if the issue is with my GPU since I only got a new power supply two months ago. My system info is AMD Ryzen 3 3100, 3 cores, 16GB RAM. It's possible something else is causing the crashes.

I'm not certain if your PSU wasn't informed before installation.

I’m considering sending it back to the original person who made it and asking them to inspect it. I also have a one-year warranty on the PSU.
