From low to medium to high it becomes very clear in certain games, and in some environments. The water in the third level, for instance, performs much better on higher settings than on low or medium. It largely depends on the game and the environment, but many settings usually show minor differences that significantly affect performance. For ultra, it's simply a "tank performance, change nothing else" option. Don't use ultra unless your FPS is already at its maximum... Ultra is just unnecessary.
