It's a unique CS:GO feature: FPS is naturally lower than what you set with fps_max. Just set it to 200 and it should work. Don't push beyond 144 fps on your 144Hz screen.

They seem to depend on frame timing, which affects accuracy. Locking at 144 keeps it stable. But why does it matter if you can't be sure?

Adjust fps_max to double your refresh rate plus one. With a 144Hz monitor, it feels noticeably smoother than limiting it to 144.

Have you considered connecting the headset to the rear of your PC or checking the front connections? The coilwhine you hear through your headset might be coming from the back plugs.
