You can play GTA V and Sekiro at ultra settings in 1080p with stable 60fps, but near Trinity Tower in Fallout 4 you experience around 26 fps. Around the city you see 50-60fps with occasional lag spikes. Outside the city performance remains solid at 60fps. Your GPU and CPU usage stays within limits, but something is affecting gameplay near the city.

The game relies on an old, outdated engine that bethesda continuously updated with patches and fixes, leading to some suboptimal performance.
It appears you're not alone in encountering this issue:
It might be connected to shadow-related problems in the game, and certain mods have been tried to resolve it, as discussed in various forums.

With a Nvidia graphics card and a Freesync display that supports it, my game performs smoothly at 30fps when Freesync is on and 60fps at 1440p with it off. When the game was released, I tested it with an i7 2600k and a GTX 980 ti at 60fps in 4k resolution. After all the DLCs were added, I could only keep 60fps at 1440p. Using mods only made things worse. Now my modified game is having trouble keeping 60fps in 4k with the current hardware.

This should be your initial game from Bethesda.
