You're seeing screen tearing on your RX 5700 XT with Freesync enabled and VSync turned off. I've tried updating drivers and using a 144Hz monitor, but the issue persists—only certain parts of the screen are affected. It seems Enhanced Sync or similar options aren't available in AMD Radeon software. Any suggestions?

Consider playing the game in Borderless windowed mode. The tearing problems from G sync have been resolved with Freesync, though it still doesn’t work consistently—only Skyrim seems to be affected.
