Hello, for your MacOS High Sierra installation on an iMac 2012 with 8GB RAM and 1TB HDD, the system is reporting an error during the update of system extension information. You've already tried formatting the hard drive and reinstalling, but it didn't work. Consider checking the disk health, ensuring proper installation media, and verifying your macOS version compatibility. If issues persist, contact Apple Support for further assistance.

Have you experimented with another USB drive and a new macOS installer? It might be the existing installation media. (Could have worked if it had)

I just discovered the issue. I put Mac OS on an external drive, then installed the driverDx app. It reported my internal hard drive is failing. Thanks for your assistance.
