Error code 69 detected – IOH DXE initialization failed.
Hey everyone, just wrapped up assembling my new PC after a long break—about eight years! But now it keeps failing to boot and restarts repeatedly, even though the LED display always shows 69 and the four LED lights flash the CPU for a short while before shutting down. I’ve also noticed the CMOS is stuck at 69, and the orange LEDs light up during startup. Despite checking all connections, reseating components, and trying different RAM setups, nothing seems to work. Any advice or insights would be super helpful. My build includes a Haf 700 case, Gigabyte Z790 AORUS Master X motherboard (rev.1), an I9 14900K CPU, Arctic Liquid Freezer II 420 A-RGB RTX 4090, and a TUF DDR5 32GB 7200MHz RAM setup with a Crucial T500 PCIe NVMe SSD, 2TB M.2 SSD, and a PowerEdge TF3 1200W cooler. I also have a Thermaltake P3 1200W power supply. I’ve uploaded a video showing the issue—check it out here: https://youtu.be/Dg4DDp0v4NA?si=_Y5npCLvkL-uZWhr. I’ve gone through all troubleshooting steps, but nothing resolves the problem. Thanks for any help!
Challenging decision! Good setup! "I've tried this with and without the GPU installed" — meaning using HDMI on the IGPU? With at least one reboot after each big change? Sounds like everything was ready, including RAM. Did you hook up a fan to the CPU fan port? Attempted to move the GPU outside the case, used just one stick of RAM, no extra cables. Verified Front Panel Header connections? Removed CLR_CMOS and Reset Jumper or confirmed they were gone. Also checked the battery? (Apologies if this seems obvious) Updated January 11, 2024 by leclod
The only connections besides the power cable and USB stick were an HDMI and DisplayPort cable to the GPU, and a DisplayPort cable to the board ret op—still no display. I removed them again. Just a rough note: this was the worst experience I've ever had with a 69.
I've checked everything I could with the GPU installed. Then I saw a discussion on another forum suggesting trying without a GPU and using the built-in one. Even without a display connected, it behaves the same. Everything seems to be working normally except for the BIOS update to version F5. I've tested all setups outside the case—just using the 24-pin M.2 power cable, two 4+4 power cables, and the AIO CPU cooler plugged into the grey port. I'm unsure what you meant by checking the front panel header connections, but I'd appreciate any advice. I've booted without them yet, thanks to the power switch on the board, and still got the same result. I haven't removed any jumpers or altered anything. I've been working on this for nearly two weeks, and it's really frustrating since I'm close to getting a new PC. Any help would be greatly appreciated. Also, these photos aren't great, but I've taken some of the board out so you can see if I made any mistakes—I'm hoping I didn't do something wrong.
Do those two unexpected connectors truly represent the two 4+4 PSU connectors? Are they placed correctly?
My description didn’t come across well. Take a look at the first image from my previous post showing the greyish-white header, which links to the third picture. Yes, the fans spin on the cooler—connecting another fan works too. Thanks for the feedback!
You're asking if the motherboard might not boot without a properly connected CPU cooler fan spinning, and you want to confirm whether your next message was received.
They clearly mention a CPU on the connectors, and that’s exactly where I connected them. I’ve also tried inserting them into the port on both sides, but the outcome remains the same. Thanks for the suggestion!
