I ran Fallout 4 on maximum settings with stable performance near 50 FPS, but recently I’m seeing about 30 FPS at lower settings in 1080p, while higher settings drop to around 30 FPS.

Overclocked GTX 960 4 GB with i5 6400 regularly, but it crashes often and I also use mods.

Check frequencies and temperature in something like GPU Z or Afterburner. If it is below 1000 MHz, then you've got a problem. My recommendation is to uninstall the drivers and reinstall new ones. That worked for me when I had this problem.

If a patch was applied before problems arose or if drivers were updated, it's best to go back to the previous version of the software. Also, verify whether any unusual behavior is occurring with the GPU clock speeds and power consumption.
