The game is running slowly. Updating the drivers helped somewhat, but it keeps stuttering. Consider other solutions.

I used the Toshiba Qosmio X300 with Core 2 Extreme QX9300 and Nvidia GeForce 9800M GTS, 4GB RAM. The game ran smoothly on Helios 300 (11900H/RTX 3060/16GB) without any stuttering, but I still couldn't reach the maximum settings.

FusionFix could be useful.
Consider starting the game from the executable in the GTA 4 folder. Set FPS to 45; the cutscenes ran smoother because they were at 30fps, but 30 is too low for gameplay. 45 is acceptable.
Stutters decrease as you go deeper into the game.

I increased my RAM from 4GB to 8GB. This resolved the stuttering and significantly improved frame rate. I'm now achieving about 35 FPS at high settings.
