You’ll need a newer processor to make Dark Souls 3 work smoothly. Based on your current setup—Intel Athlon II X2 215, 4GB DDR2 at 800MHz, and an older graphics card like the HD 6570—upgrading to something like an Intel Athlon X3 450 or a Phenom II X4 955 should resolve the performance issues. These newer chips offer better power efficiency and higher clock speeds, which will help achieve stable frame rates without the sluggishness you’re experiencing.

From the images, your processor is running very heavily. I suggest updating it first. Once you confirm your motherboard and cooler can handle it, consider switching to the Phenom model. It uses a 125-watt component.

Could you please clarify the details of the upgrade you're referring to? I need more information to provide an accurate answer.

Consider saving some funds and purchasing a reliable computer outright. Gradual upgrades aren't worthwhile unless you can find used options at very low prices.

Yeah, perhaps you're correct, but those CPUs are around 30 dollars each, which isn't a big deal.

What model are you using? Given these specifications, I wouldn’t suggest a new CPU since I’d expect a faulty board that would fail if you installed a Phenom II 955 and tried gaming on it. The graphics card is also unsuitable for gaming and better suited for office tasks. Regarding the power supply, could you provide an image of the label if you’re unsure about the manufacturer or model?

I have a board model m4n78 SE 400w power supply with a stock cooler, and I needed to swap out the fan for a cheaper alternative.

Check the maker and model of the device. A 400W rating doesn't indicate much about your vehicle's condition. It doesn't reveal much about the power supply unit, which is important. Many companies customize their products in various ways.
