In your case, you can connect to the VPN on either your phone or the device getting the hotspot—it doesn't matter which one.

When the VPN is enabled on your phone, all data sent from your device goes through the VPN server. Because every connected device uses your phone’s hotspot, their traffic is also directed via the VPN. You don’t have to keep it active on any other gadgets linked to the hotspot.

You don't have to link the VPN on both devices. It might be helpful to reach out to Nord VPN support and inquire about this.
