It's a solid improvement, really. Running at medium settings with the resolution slider adjusted and AA enabled, plus lower shadows, gives me around 40fps. Not perfect, but it's better than before.

What adjustments? When I start the game, I switch Windows to basic mode (not sure if it makes much difference) and disable the rain meter because it uses 30% of my CPU when idle. I also set the game to high priority in Task Manager (still unsure about the benefit, but it seems to speed up loading) and modified a setting in the Nvidia control panel. I’m using the launch option for an SM4 processor, which RagnarokDel mentioned is newer in this discussion.

You're experiencing strong effects at mid-40s with similar configurations to your friend. -sm4 reports readings in the low 100s.
