On borderless or windowed modes, the refresh rate aligns with the desktop's settings. In fullscreen, does Vsync remain enabled or disabled?

V sync is active at full 60fps, not a reduced 30fps "half" version. The issue isn't missing the 60fps, but after switching tabs, the frame rate stabilizes near 60fps.

I discovered the vsync in this game is terrible. Just switch it off and you'll have a better life.

You faced the in-game double buffered Vsync limiting your frames to 30 once it fell below 60. As it approached 30, the GPU caught up and restored the cap to 60—until another frame slipped, then it dropped back to 30. This cycle repeated until you felt a headache. Ideally, D3DOverrider should be active during gaming. Enable triple buffering and Vsync constantly, and disable in-game Vsync. Smooth performance remained even when your frame rate fluctuated between 55 and 60. The main drawback was slight input lag, but avoiding tearing or the jarring back-and-forth between 30 and 60 was better. When you switch tabs or return to the game, Vsync usually shuts off automatically.

GTA V lacks triple buffering support, which explains the issue you're facing. The reported behavior is likely a typical workaround. Another solution is to switch to Borderless-Fullsreen.
