In full protection history there are actually three threats listed, though they were removed recently—two weeks ago. This might be what you’re seeing. As for why now, it’s possible something changed. Regarding the “failed!” message, it likely means the system detected activity but couldn’t confirm removal successfully. You mentioned removing files quickly because of modding, which is common since modders sometimes take down their content for various reasons. Virustotal and Malwarebytes didn’t find anything, so no malware detected. It’s confusing, but the system seems to be handling it.
