I've faced this issue for over four years, yet a solution remains elusive.
The situation I'm discussing stems from the fact that I've completely overhauled every part of my PC—just upgraded to a 4070ti—and it still persists.
Current details:
Processor: Ryzen 3700x (also tested 3600 and 5800x on a B550 board)
Graphics: MSI B450 Tomahawk (all BIOS versions tried)
Memory: 8GBx2 3600 RAM (previously had 3000mts, no change, multiple slots used)
GPU: 4070ti Gigabyte Eagle (previously MSI 2070 Gaming Z, same problem)
Storage: Samsung 850 EVO, Crucial drives (2TB, 4TB, 1TB), all with ample free space
Monitor: Acer XV282 4k 144hz (also tested LG 32" 1440p and Iiyama 34")
I've used MSI Afterburner for thermal checks, usage monitoring, and everything appears normal.
GPU temperatures stay around 55°C on the new unit; the older 2070 reached about 68°C.
CPU operates between 60-70°C during gaming, with a clean system and no dust.
Frame capture tools like CapFrameX show only minor stutters, which can't be measured.
The concern lies when Dota is locked at 144fps—it functions fine, but any drop below a few fps disrupts the experience.
I've explored all settings under gsync, vsync, fast sync, yet nothing changes.
The Gsync logo appears in-game indicating it works, and the Nvidia Pendulum test also confirms functionality.
The only untested possibilities are an Intel motherboard/CPU upgrade or switching to AMD GPU, or using a different monitor.
I've experimented with various power supplies and even tried using a controller, but no success.

Dota 2 stands out as the most problematic, remaining smooth at 144fps but failing even a single fps drop.
It's frustrating since the issue seems consistent across all settings, yet no one else reports it.
I'm running out of patience and hope for a resolution soon.
Update your post with complete hardware details and operating system information.
Power supply: make, model, wattage, age, condition (original to build, new, refurbished, used)?
Storage devices: make, model, capacity, current usage?

Run Task Manager and Resource Monitor to monitor performance.
Use each tool separately but keep one open at a time.
Power on, launch the application that causes stuttering, and remain idle for a while.
After that, start a game or application and continue observing.
Identify which resource is being consumed and the percentage involved, especially during stuttering.
An additional utility could be Process Explorer (Microsoft, free).

The stuttering occurs whenever the game isn't running at 144fps, though it doesn't happen all the time. I'm focusing on understanding Dota better, especially trying to figure out what's causing it.
For example, Dota stutters at 144fps when playing at other speeds. I enabled Nvidia Reflex, which limited the frame rate to 138fps and eliminated the stutter.

Stuttering refers to a brief shortage of a crucial component.
It often stems from CPU issues, but can also result from insufficient RAM. Many people are using 32GB these days.
To determine your memory needs, open Task Manager/Resource Monitor/Memory tab and examine the hard page fault/sec column.
When stuttering occurs, look for hard faults.
If you notice any values above zero, adding more RAM could help.
Are you running any other applications at the same time?
Discord or streaming? Maybe Chrome tabs are open?
Have you scanned for malware?
How is your single-threaded CPU performance?
This is typically what games require.
Run the cpu-Z benchmark on your 3600X.
You should see a score around 515:

If budget isn’t a concern, think about upgrading to an I9-13900K or even the I9-13900KS.
I'm getting 521 in CPU-Z in Single Core. I've tried a 5800x on the same and a different mobo as well, same thing.
Unfortunately no change if I run nothing in the background. My pc usually only ever has steam, geforce experience and maybe a few chrome tabs in the background.
I get 0 Hardfaults/sec in Resource Monitor.
Used Malware Bytes & Defender, no malware or other signs of malware either.
If I play Battlefield V as another example, the frametimes appear fine, but can still see this stutter.
Like it skips a frame or something. Same as the Youtube videos above.
SSD, I've also tried various games on all my drives, same problem.

Have you checked if the CMOS was cleared after each bios update? If not, you definitely need to verify that.

It's true, my b450 tomahawk has always had some issues with BIOS updates, needing a CMOS reset each time.
I also experimented with a b550 Gigabyte motherboard later on, thinking it might be the problem, but it didn't help. Plus, I tried the b550 with a 5800X.
