Your display settings at 4k are causing performance problems. It seems you're likely limiting your frame buffer space (about 3.5GB of VRAM) and dealing with a huge pixel count to process.

Certainly, the 2x SSAA boost provides roughly 40-55fps for lows and highs on my 1100mhz 290X, which suggests the problem might be related to maintaining 60fps when using Vsync.

I might be mistaken, though the monitor settings you mentioned are a 16:9 format which isn't common. I only notice two displays using that size—one Dell and another Samsung from around 2009. Is this your standard resolution? If yes, it's actually higher than 4K with double scaling.

It's correct, as I mentioned before. Although Monday I'm getting a new monitor, I recently bought a Ryze. I was using a less reliable one that ran at 60 frames now that it's not adaptive, but I'm experiencing a loud squeaky noise from my setup, probably the GPU.
