Hello, My computer has been crashing every three days while playing games. I tried all sorts of fixes—changing RAM slots, updating BIOS, refreshing graphics and drivers, updating Windows—but the issue persists. The screen goes black after just a few minutes, the game audio loops, and eventually the game crashes. Windows shows a notification that the application is blocked from using the graphics card. Additionally, benchmarking and stability tests succeeded only on GPU and PC crashes occurred during gameplay.

OP, do you have plenty of RAM? Have you attempted to run it on a single stick to check stability? Also, consider downclocking the GPU to determine if the GPU is the problem.

I attempted to operate it with just one RAM stick and encountered the same issue.
For your EVGA GTX Titan x sc, how much should I lower the GPU clock speed?

I would start with 100mhz initially just to ensure safety, or consider increasing the voltage if possible.

I attempted to adjust the GPU clock and memory timings, increased the voltage by +6 mV, and it appears the issue is resolved.

It seems like you'd prefer to revert to auto mode and adjust the voltage offset to around 25mV or similar, aiming for maximum stable speed.

I adjusted the frequency to auto, configured the voltage to +24, and the GPU experienced another crash.

Yes, higher than stock voltages may lead to stability problems over time, though the titan X isn't very old. EVGA offers a 3-year warranty, so I might consider sending it back if possible.
